The following summarized guidelines for the management of clostridial myonecrosis are prepared by our editorial team based on guidelines from the Korean Society of Infectious Diseases (KSID/KDA/KOA/KSC 2017) and the Infectious Diseases Society of America (IDSA 2014).
Antibiotic therapy: as per KSID 2017 guidelines, administer combination therapy with penicillin and clindamycin as the definitive antibiotic therapy.
Hyperbaric oxygen therapy: do not offer hyperbaric oxygen therapy in patients with gas gangrene because of a lack of benefit and the risk of delayed resuscitation and surgical debridement.
Surgical debridement: as per KSID 2017 guidelines, perform early surgical debridement of involved tissue and obtain a continued visual evaluation and remove the infected area.
