- All other diagnoses: 1 point

Stigmata of hemorrhage are assigned scores based on severity:
- No stigma or dark spot on ulcer: 0 points
- Presence of blood in the upper GI tract, an adherent clot, or visible/spurting vessel: 2 points

The total score is calculated by summing the scores from each category, resulting in a score between 0 and 11. This total score is used to assess the patient's risk of rebleeding and mortality:

- A total score of 0 indicates a 4.9% risk of rebleed and 0% mortality.
- A score of 1 predicts a 3.4% risk of rebleed and 0% mortality.
- A score of 2 corresponds to a 5.3% rebleed risk and 0.2% mortality.
- A score of 3 raises the risk of rebleed to 11.2% and mortality to 2.9%.
- A score of 4 foresees a 14.1% rebleed risk and 5.3% mortality.
- A score of 5 increases the risks to 24.1% for rebleed and 10.8% for mortality.
- A score of 6 results in a 32.9% rebleed risk and 17.3% mortality.
- A score of 7 predicts a 43.8% rebleed rate and 27% mortality.
- A score of 8 or more suggests a 41.8% risk of rebleed and 41.1% chance of mortality.
