When to use
Seizure (metabolic, infectious, or drug causes excluded)
Recent onset seizure
Psychosis (uremia and drug causes excluded)
Altered ability to function in normal activity due to severe disturbance in the perception of reality, including hallucinations, incoherence, marked loose associations, impoverished thought content, marked illogical thinking, and bizarre, disorganized, or catatonic behavior
Organic brain syndrome (metabolic, infectious, or drug causes excluded)
Altered mental function with impaired orientation, memory, or other intellectual function, with rapid onset and fluctuating clinical features, inability to sustain attention to the environment, and ≥ 2 of the following: perceptual disturbance, incoherent speech, insomnia or daytime drowsiness, and increased or decreased psychomotor activity
Visual disturbance (hypertensive, infectious, or drug causes excluded)
Retinal changes of systemic lupus erythematosus, including cytoid bodies, retinal hemorrhages, serous exudates or hemorrhages in the choroid, and optic neuritis
Cranial nerve disorder
New onset sensory or motor neuropathy involving cranial nerves
Lupus headache
Severe, persistent headache; may be migrainous but must be nonresponsive to narcotic analgesia
Cerebrovascular accident (arteriosclerosis excluded)
New onset cerebrovascular accident
Vasculitis
Ulceration, gangrene, tender finger nodules, periungual infarction, splinter hemorrhages, or biopsy or angiogram proof of vasculitis
Arthritis
≥ 2 joints with pain and signs of inflammation: tenderness, swelling, or effusion
Myositis
Proximal muscle aching/weakness associated with elevated creatine phosphokinase/aldolase, electromyography changes, or a biopsy showing myositis
Urinary casts
Heme-granular or red blood cell casts
Hematuria (stone, infection, or other causes excluded)
> 5 red blood cell/high power field
Proteinuria
Urine protein > 0.5 g/24 hours
Pyuria (infection excluded)
> 5 white blood cell/high power field
Rash
Inflammatory-type rash
Alopecia
Abnormal, patchy, or diffuse loss of hair
Mucosal ulcers
Oral or nasal mucosal ulcers
Pleurisy
Pleuritic chest pain with pleural rub/effusion, or pleural thickening
Pericarditis
Pericardial pain with ≥ 1 of the following: pericardial rub, effusion, or electrocardiogram or echocardiogram confirmation
Low complement
Decrease in CH50, C3, or C4 below the lower limit of normal for laboratory testing
Increased deoxyribonucleic acid binding
Increased deoxyribonucleic acid binding by Farr assay above the normal range for laboratory testing
Fever (infectious causes excluded)
Temperature > 38 degrees C
Thrombocytopenia. Exclude drug causes.
Platelets < 100,000/mcL
Leukopenia (drug causes excluded)
White blood cells < 3,000/mcL
